Summary

Membership in the Uncorked Wine Consulting, Inc. Wine Club is an annual fee of $500.00 per member. While you are in good standing with the Uncorked Wine Club, you will have the ability to purchase all wines[1] at 10% over wholesale cost. Wine orders will be shipped and club member will be responsible for the shipping fees.

You Must Be 21

Wine may only be purchased by, shipped to, and received by adults 21 years of age or older.

Commitment Requirement

A minimum of commitment of 12 months is required.

Cancellation Policy

All cancellations must be received by Uncorked Wine Club in writing, by fax or by electronic mail, originating from the address of record (listed on this sign-up form), at least 5 business days before the annual renewal.

Modification Policy

Uncorked Wine Consulting, Inc. reserves the right to change the policies of the Uncorked Wine Club. Members will be notified of any changes by e-mail, U.S. postal mail, or telephone before any further transactions.

Agreement

I have read and agree with all the Terms & Conditions for membership in the Uncorked Wine Consulting Wine Club, including the Commitment Requirement. I authorize Uncorked Wine Consulting, Inc. to use my credit card for this purpose until written notice for cancellation has been received, according to the Terms & Conditions listed above.
