The Wine of the Month Club is perfect for budding wine lovers and experts alike!   This program is completely customized to you!  From duration of Membership (6 or 12 months), two pricing tiers ($30 and $60 per month, plus shipping) to the ability to specify the combination of wines you desire to receive each month!  Uncorked's Wine of the Month Club will provide exposure to varying varietals and wine regions. 

Membership Includes:

• 6 or 12 months of monthly wine deliveries, two bottles per month.  Your shipment will consist of your choice of two white wines, two red wines or one white and one red wine each month. 
• Complete tasting notes and food pairing suggestions for each of the wines.
• Opportunity to have exposure to wines that are NOT available to grocery stores or wine shops.
• Opportunity to reorder wines from shipments at discounted pricing.
• Access to your own personal Wine Consultant who will build a personal relationship with you and readily suggest wines that will compliment your palate profile.
• No more guessing, no hassle, and no more bad wine!

Summary

Membership in the Uncorked Wine Consulting, Inc. Wine of the Month Club is charged monthly based on the desired tier of the club.  While you are in good standing with the Uncorked Wine Club, you will have two bottles of wine shipped to your residence on a monthly basis. Included in your package will be tasting notes as well as suggested food pairings.

Wine may only be purchased by, shipped to, and received by adults 21 years of age or older.

Commitment Requirement

A minimum of commitment of 6 months is required.

Cancellation Policy

All cancellations must be received by Uncorked Wine of the Month Club in writing, by fax or by electronic mail, originating from the address of record (listed on this sign-up form), at least 5 business days before the annual or monthly renewal.  In the event that you wish to cancel your membership before the required commitment of 6 months is met, your credit card will be charged the remainder of your membership fee.

Modification Policy

Uncorked Wine Consulting, Inc. reserves the right to change the policies of the Uncorked Wine Club.  Members will be notified of any changes by e-mail, U.S. postal mail, or telephone before any further transactions.

Agreement

I have read and agree with all the Terms & Conditions for membership in the Uncorked Wine Consulting Wine of the Month Club, including the Commitment Requirement.  I authorize Uncorked Wine Consulting, Inc. to use my credit card for this purpose until written notice for cancellation has been received, according to the Terms & Conditions listed above.
